Individuals interested in UCCS services complete
an “Application/ Informed Consent for Services”
form and are scheduled to complete an initial needs
assessment. Individuals may also utilize the open
access intake process available each Wednesday.
Once needs are identified, a plan for services is
developed. Should services offered by UCCS not
be beneficial, the individual is referred to an agency
that may be more suitable.
UCCS utilizes an interdisciplinary treatment
team in assigning cases to the most appropriate
